"A" Hospital Safety Score (SM) by The Leapfrog Group for 2014
Bozeman Deaconess Health Services is pleased to be recognized for the second
time with an A Hospital Safety Score(SM) by The Leapfrog Group, an independent
national nonprofit. Bozeman Deaconess is one of only two hospitals in
Montana to receive a consecutive "A" rating from this leading
healthcare quality and safety organization, putting us in the top 25%
in the nation.
The Hospital Safety Score(SM) was calculated under the guidance of The
Leapfrog Groups Blue Ribbon Expert Panel using publicly available data
on patient injuries, medical and medication errors, and infections. U.S.
hospitals were assigned an A, B, C, D, or F for their safety.

Bozeman Deaconess has been recognized by Healthgrades as one of America’s
100 Best Hospitals for Pulmonary Care. In its study of patient outcomes,
Healthgrades, a leading online resource helping consumers compare physicians,
hospitals and care, found Bozeman Deaconess Health Services to be among
the best performing hospitals nationally for pulmonary care.

Bozeman Deaconess Health Services has received the Healthgrades Pulmonary
Care Excellence Award for 2015, for the second year in a row (2014-2015),
and ranked among the top 5% in the nation for Overall Pulmonary Services,

BDHS is named a Five Star Recipient for treatment of GI Bleed for two years

Bozeman Deaconess Hospital has been named a 2014 Healthstrong Hospital
by iVantage Health Analytics, a leading healthcare advisory and business
analytic services company that provides comparative healthcare analysis
of market, clinical, operational and financial data to help hospitals
make better strategic decisions and improve performance.
Based on measurements of ten equally rated pillars of performance at 4,299
acute care hospitals, BDH received a Hospital Strength Index rank of 99.8.
That score placed BDH into iVantages top tier of 547 Healthstrong hospitals.
iVantage calls its Hospital Strength Index the industrys most comprehensive
ratings system based on publically available data. The ten pillars are
grouped into three categories: Risk (Competitive Strength, Competitive
Intensity, Market Size and Growth, Population Risk), Value (Cost, Charge,
Quality, Outcomes, Patient Perspectives), and Performance (Financial Stability).

By demonstrating compliance with The Joint Commission‘s national
standards for health care quality and safety, Bozeman Deaconess Hospital
has earned the Joint Commission’s Gold Seal of Approval™.

Bozeman Deaconess Health Services has been named a Top Performer on Key
Quality Measures® by The Joint Commission, the leading accreditor
of health care organizations in America, for exemplary performance in
using evidence-based clinical processes that are shown to improve care
for certain conditions.
One of only 1,099 hospitals in the U.S. earning the distinction, BDHS was
recognized for its achievements in heart attack, pneumonia and surgical
care. Bozeman Deaconess achieved cumulative performance of 95 percent
or above across all accountability measures, which represent evidence-based
practices such as giving aspirin at arrival for heart attack patients
or giving antibiotics one hour before surgery.

Community Hospitals by Becker’s Hospital Review, 2013
Bozeman Deaconess Hospital has been named of the nation’s “100
Great Community Hospitals” by Becker’s Hospital Review, a
publication offering business and legal news and analysis relating to
hospitals and health systems. The award was based on the hospital’s
rankings and awards from influential healthcare analysis organizations,
quality of care and services provided to patients.
Bozeman Deaconess Hospital Named One of the Nation's 100 Top Hospitals, 2013
Bozeman Deaconess Hospital was named one of the nations 100 Top Hospitals
by Truven Health AnalyticsTM, a leading provider of information and solutions
to improve the cost and quality of healthcare. Bozeman Deaconess Hospital
is one of only 20 hospitals to earn this award in the Small Community
Hospital category out of 871 compared nationwide. This is the first time
Bozeman Deaconess Hospital has been recognized with this honor.
The Truven Health 100 Top Hospitals study identifies hospitals and leadership
teams that provide the highest level of value to their communities, based
on a national balanced scorecard that measures hospital performance across
10 areas: mortality, inpatient complications, patient safety, average
patient length of stay, patient satisfaction, adherence to clinical standards
of care, and post-discharge mortality and readmission rates for acute
myocardial infarction (heart attack), heart failure, and pneumonia; cost
efficiency and overall operating performance.

The 2013 Vision Award from SightLife, a global health organization focused
on eliminating corneal blindness worldwide, goes to Bozeman Deaconess
Hospital in recognition of BDHs efforts with 27 eye donors who provided
34 sight restoring transplants in 2013.

Bozeman Deaconess Health Services has earned the Quality Achievement Award, 2013
Bozeman Deaconess Health Services has earned the Quality Achievement Award
from Mountain-Pacific Quality Health for the sixth year in a row. BDHS
was one of only three hospitals in the state to win Mountain-Pacific’s
highest award this year, a reflection of Bozeman Deaconess’ results
in improving quality performance, eliminating avoidable patient harm and
delivering exceptional patient experiences. The Quality Achievement Award
recognizes Montana hospitals for their dedication to preventing surgical
infections and ensuring patients receive high-quality care for heart attack,
heart failure or pneumonia.
Bozeman Deaconess Diabetes Center Earns National Recognition for 2013
Bozeman Deaconess Diabetes Center has earned three-year recognition from
the National Committee for Quality Assurance (NCQA). NCQA’s Diabetes
Recognition Program recognizes clinicians who have met standards demonstrating
delivery of high-quality care to patients with diabetes.
Bozeman Deaconess Diabetes Center providers Michael Vlases, MD; Sebastian
White, NP; and Leslee Jones, PA-C, were linked specifically to this honor
for doing their part to advance quality in health care. This is the second
time the center has earned this recognition.
Cancer Center is awarded a full three-year accreditation, 2012
Bozeman Deaconess Cancer Center was awarded a full three-year accreditation
with commendation from the American College of Surgeons/Commission on
Cancer in December, with particular citation for compliance in outcomes
analysis, prevention and early detection, staff education and quality
improvements, among other areas.
By undertaking this voluntary evaluation, the Cancer Center demonstrated
its commitment to quality care, ongoing improvement, and public accountability
for the care and services provided.
2013 Quality Achievement Award from the American Heart Association
Bozeman Deaconess Heart Center has earned a 2013 Quality Achievement Award
from the American Heart Association. Bozeman Deaconess was recognized
as a Mission: Lifeline STEMI Receiving Center Silver Performance Achievement

Bozeman Deaconess Health Services Joint Replacement program received a
five-star rating in 2013 from HealthGrades, the leading provider of physician
and hospital information for consumers, for the second year in a row.
Bozeman Deaconess was the recipient of the HealthGrades Joint Replacement
Excellence Award in 2012, when it also was ranked among the top 10% of
hospitals in the nation and ranked #1 in Montana for joint replacement.

For the fifth year in a row and the eighth time, Bozeman Deaconess received
the 2012 Mountain-Pacific Quality Achievement Award, the highest award
possible. Bozeman Deaconess was recognized for its commitment to ensuring
that patients receive the highest quality of care in certain areas. Bozeman
Deaconess has previously won the award in 2004, 2005, 2006, 2008, 2009,
2010 and 2011.
Five-Star Rated for Coronary Interventional Procedures, 2011
- HealthGrades awarded Bozeman Deaconess Heart Center a five-star rating
for three years in a row (2009-2011) for quality of its coronary interventional
- Bozeman Deaconess Heart Center is also ranked among the Top 5 in Montana
for Coronary Interventional Procedures in 2009, 2010 and 2011.
First in Montana for NCQA Level III Medical Home Recognition, 2011
Four Bozeman Deaconess Health Group primary care practices have received
recognition from the National Committee for Quality Assurance (NCQA) Physician
Practice Connections—Patient-Centered Medical Home (PPC-PCMH) program
for using evidence-based, patient-centered processes that focus on highly
coordinated care and long-term participative relationships.
Notably, the Bozeman Deaconess Health Group clinics are the first in the
State of Montana to earn Level III Medical Home Recognition. Practices
receiving recognition are Bozeman Deaconess Bridger Internal Medicine,
Bozeman Deaconess Hathaway Internal Medicine, Bozeman Deaconess Internal
Medicine Associates, and Bozeman Deaconess Family Medicine and Pediatrics,
Exceeding Patient Expectations Award, 2011
As evidence of our commitment to quality and continued improvement, Bozeman
Deaconess Hospital is being awarded an Avatar International 2011 Exceeding
Patient Expectations Award.
Designated Blue Distinction Center for Knee and Hip Replacement, 2011
Bozeman Deaconess Hospital has received designation as a Blue Distinction
Center for Knee and Hip Replacement by Blue Cross and Blue Shield of Montana.
Blue Distinction designations are awarded to facilities that have demonstrated
a commitment to quality care by meeting objective, evidence-based thresholds
for clinical quality and safety developed with input from expert clinicians
and leading professional organizations.
Five-Star Award for Community Value, 2007-2010
For the fourth year in a row, Bozeman Deaconess has earned a Five-Star
Award for Community Value for scoring in the top 20% of over 1500 similar
hospitals in an evaluation by the national healthcare organization Cleverley
+ Associates. Hospitals with scores in the top 20% are designated as "Five-Star"
facilities and the measurements suggest hospitals operating with a high
degree of community value are those that are quality of care, low cost,
low charge, and use a strong financial position to reinvest back into
the provision of care at the facility.
Bozeman Deaconess was one of two Montana hospitals recognized for the award.
NCQA Diabetes Management Accreditation, 2010
Bozeman Deaconess Diabetes Center has been recognized by the National Committee
for Quality Assurance (NCQA) for diabetes care. The Center earned three-year
NCQA recognition for its overall high quality of diabetes care. Patients
cared for at the Diabetes Center exceed standards for blood glucose control
and surveillance for complications including nerve damage, eye damage,
kidney damage, and control of cholesterol and control of blood pressure.
