Comprehensive Diabetes Management Living with diabetes presents day-to-day challenges. Left untreated over time, diabetes can cause serious health problems that affect many systems of the body from head to toe. With diligent self-management, however, individuals may be able to prevent or delay these health problems. Bozeman Deaconess Diabetes Center offers comprehensive care and support to help individuals of all ages including pediatrics. We work closely with patients, families, primary care physicians, other referring physicians and healthcare professionals to meet the complex needs of individuals with diabetes. Our patient-centered program focuses on clinical, educational and psychosocial needs. Ultimately, our goal is to help patients minimize the devastating complications of diabetes through diligent disease management. Our Services Bozeman Deaconess Diabetes Center provides a full complement of clinical and support services aimed at helping individuals manage this complex and chronic disease. We work with adults and children of all ages, families and pregnant women (including women with diabetes related to pregnancy). All services require a physician referral. Our services include:
With a strong focus on educating and empowering patients to manage their symptoms, Bozeman Deaconess Diabetes Center has earned recognition from the American Diabetes Association for quality education in diabetes self management. In addition, The medical providers at the Bozeman Deaconess Diabetes Center have received a certificate of recognition from National Committee for Quality Assurance (NCQA) and The American Diabetes Association. Our Staff The Bozeman Deaconess Diabetes Center is staffed by a multidisciplinary team of health professionals who are focused exclusively on the needs of people with diabetes. Drawing from their diverse expertise, this team helps patients to address the many physical, social, emotional and lifestyle complexities of living with diabetes. The diabetes care team centers around one crucial team member-the patient. We view each patient as the vital partner in his or her own care. In addition to a board certified physician, our professional staff includes a nurse practitioner, a physician's assistant, Certified Diabetes Educators, registered nurses and registered dietitian. Physician Referral Required Most health insurance plans, including Medicare, provide coverage for diabetes care and education when services are prescribed by your physician. Educational Classes Comprehensive Diabetes Education: Whether you are recentlydiagnosed or have lived with diabetes for many years, ongoing education is essential to maintaining good health. The more you know about your diabetes, the more effective you will be in controlling it. The following educational programs are designed using national diabetes education standards to help you successfully manage your diabetes. Diabetes Self-Management Class: This two-part class is for individuals interested in learning how to manage theirdiabetes, care for themselves, and control blood glucose levels. This program has been recognized by the American Diabetes Association (ADA). If you are newly diagnosed with diabetes you should start with this class. Diabetes Nutrition 101 and Counting Carbohydrates Class: Basic information is covered including when, what and how much to eat when managing diabetes. It also includes basic training on carbohydrate counting including hands-on practice to help understand the impact of food on blood sugar. Pre-Diabetes Class: This is a two-part class for those diagnosed with Impaired Glucose Tolerance (IGT), Pre-diabetes, Metabolic Syndrome or Insulin Resistance. Learn how and when to monitor blood sugar levels and interpret results, how food affects blood sugar, tips for weight management, healthy food choices and how to begin a safe exercise program. Additional Information Diabetes education classes are held at Bozeman Deaconess Hospital. Registration and a physician’s referral are required for insurance reimbursement. Not all insurance companies cover education services, pre-approval is recommended. Please call 406-556-5331 to register or for more information. Related Community Programs Diabetes Forum: This support/education group meets one Wednesday, per month at 12:00 pm. Please contact our diabetes center for information.
